Exceptional Support Gems are more transformative, offering an additional way to customise your build in the late game and extending character customisation even further into a build’s life.
In Path of Exile (PoE), Exceptional Support Gems refers to a specific category of endgame gems that provide powerful utility or transformative effects. As of the 3.28 Mirage League, this system has been significantly overhauled to replace the previous "Awakened Support Gem" system.
Upon defeating the Endgame Incarnation bosses in Path of Exile: Mirage, powerful new Support Gems will begin dropping from specific Atlas Bosses. These new Support gems are categorised as Exceptional, being a significant expansion of that class of Support gem.
Exceptional gems are strictly endgame rewards and cannot be purchased from vendors:
Originator's Voidstone: Defeating the Incarnation bosses for the first time rewards this Voidstone, which enables these gems to drop from bosses.
This Voidstone allows new Exceptional Support Gems to drop from many bosses across the Atlas.
They primarily drop from specific endgame encounters and pinnacle bosses.
Specific cards like The Enlightened remain a primary way to target-farm the classic cycle (Empower/Enlighten/Enhance).
With the 3.28 Mirage Content Update, "Exceptional Support Gems" now describes a broader system of over 40 new gems:

Cooldown Recovery Support is an Exceptional Support Gem. Tag: Exceptional, Support. Level: 3 (Max). Cost & Reservation Multiplier: 150%. Requires Level 76, 52 Dex, 75 Int Supports skills that have a cooldown. Cannot modify the skill of minions.

Frostmage Support is an Exceptional Support Gem. Tag: Exceptional, Cold, Arcane, Support, Spell. Level: 1. Requires Level 72, 114 Int Supports spell skills that deal damage with hits and have no reservation. Cannot support arcane skills, Brand skills, Orb skills, Vaal skills, or skills used by totems, traps or mines. Cannot modify the skills of minions.

Greater Spell Cascade Support is an Exceptional Support Gem. Tag: Exceptional, Support, AoE, Spell. Level: 3 (Max). Cost & Reservation Multiplier: 140%. Requires Level 76, 120 Int Supports spell skills that apply an effect to an area around a targeted location. Cannot support Vaal skills or skills used by totems, traps, or mines. Cannot modify the skills of minions.

Voidstorm Support is an Exceptional Support Gem. Tag: Exceptional, Support, Attack, AoE, Duration, Bow. Level: 1. Cost & Reservation Multiplier: 140%. Requires Level 72, 114 Dex Supports ranged attack skills that fire projectiles upwards. Cannot support triggered attacks, attacks used by things other than you, or which create minions.
VoidstormCast Time: 0.50 sec. Attack Damage: 140% of base. Effectiveness of Added Damage: 140% Creates a voidstorm that blinds enemies within it. Entering the storm will cause it to explode, dealing attack damage in an area.

Foulgrasp Support is an Exceptional Support Gem. Tag: Foulgrasp Support. Exceptional, Brand, Support, AoE, Spell, Lightning, Trigger. Level: 1. Cost & Reservation Multiplier: 130%. Requires Level 72, 50 Dex, 72 Int Supports brand skills that can be recalled.
FoulgraspCooldown Time: 1.00 sec. Cast Time: 1.00 sec. Critical Strike Chance: 6.00%. Effectiveness of Added Damage: 110%. Summons forth a field of grasping hands, each of which clutches an orb of lightning. When an enemy is near a hand, the orb it clutches will detonate to deal lightning spell damage.

Greater Multistrike Support is an Exceptional Support Gem. Tag: Exceptional, Support, Melee, Attack. Level: 3 (Max). Cost & Reservation Multiplier: 160%. Quality: +20%. Requires Level 76, 75 Str, 52 Dex Supports melee attack skills, making them repeat three times when used, targeting a random enemy each time. Cannot support Vaal skills, channelling skills, travel sskills, retaliation skills or triggered skills.

Summon HivebornCooldown Time: 5.00 sec. Cast Time: 1.00 sec. Summons a horde of Hiveborn Breach monsters to do your bidding. These summoned monsters have all their physical damage converted to cold.

Hextoad Support is an Exceptional Support Gem. Tag: Exceptional, AoE, Duration, Support, Hex, Spell, Chaos. Level: 3 (Max). Cost & Reservation Multiplier: 160%. Quality: +20%. Requires Level 76, 120 Int Supports non-aura hex curse skills.
Bursting ToadCast Time: 1.00 sec. Critical Strike Chance: 5.00%. Effectiveness of Added Damage: 400% A toad leaps towards enemies and explodes, dealing spell damage in an area.
