You must kill the specific Breach mini-boss, Vruun, Marshal of Xesht, while your character is under the effects of Delirium fog. Vruun, Marshall of Xesht is a unique monster that can be sometimes be found in a stabilised Unstable Breach. He is guaranteed to spawn when using a Breach Scarab of the Marshal. Use a Delirium Orb for constant fog.
